Pour éponger les dettes de son frère, Tommy accepte d'aller livrer en mains propres un colis à l'Allemand pour le compte de Big Doug, un des plus gros mafieux de la région. En réalité, Doug cherche à se débarrasser de Tommy. L'Allemand a un besoin urgent de transplantation et Tommy est un des seuls au monde à avoir le même groupe sanguin que lui. Tommy réalise alors qu'il est tombé dans un guet-apens et qu'il est le package. Seul contre tous et prêt à tout pour sauver son frère, il va pulvériser tous ceux qui se mettront en travers de sa route...
